The terms of Denplan Care and Denplan Essentials with our practice, including what each plan provides and our attendance policy.
Denplan Care covers the routine and restorative treatment necessary to maintain your oral health, provided on clinical prescription for a fixed monthly fee.
Good to Know
The key points of your Denplan Care membership at a glance.
Missed appointments are chargeable
We require at least 48 hours' notice to cancel or rearrange an appointment. A missed appointment fee is payable if you do not attend or give less than 48 hours' notice.
Regular attendance is required
You must attend your scheduled examinations and complete the treatment your dentist prescribes. Treatment needed as a result of non-attendance may be charged separately.
Your fee band may be reviewed
If your oral health declines, your plan may be re-banded to a different fee category, with one month's written notice.
Treatment is provided on prescription
Denplan Care does not entitle you to a specific amount or type of treatment. Care is provided as clinically necessary, at the discretion of your dentist.
1. Dental Fitness
Acceptance onto Denplan Care is subject to an assessment of your oral health, which determines your fee category.
1.1 Oral Health Assessment
Prior to registration with Denplan Care at The Forge Dental Surgery, your dentist may carry out an oral health assessment to establish the current condition of your teeth and gums. This assessment forms the baseline from which your oral health will be maintained.
1.2 Assessment Criteria
Your mouth is scored across the following areas:
Existing restorations, including fillings, crowns, bridges, veneers and dentures
Bone loss around the teeth
Tooth wear, assessed using the BEWE index
Periodontal (gum) status, from stable to currently unstable
Gingival condition, from healthy gums to periodontitis
1.3 Fee Categories
The total assessment score places you in a fee category from A (a score of 0–10) through to E (a score of 141 or above). Your monthly fee corresponds to this category. Dental implants are not included in the assessment score.
1.4 Registration Fee
A one-off registration fee of £15 per person may be payable upon joining.
1.5 Re-banding
Should your oral health change, the practice reserves the right to amend your plan level, the treatment included and your monthly fee. You will be given one month's written notice of any such change.

2.5 Payment
Your monthly fee is collected by Direct Debit, administered by Denplan on behalf of the practice.
3. Attendance and Non-Attendance
Maintaining your oral health depends on regular attendance. Failure to attend may result in additional charges or termination of your plan.
3.1 Patient Responsibilities
As a Denplan Care patient, you are responsible for:
Keeping all appointments made with your practice team
Attending regular examinations at the intervals your dentist recommends
Undergoing the treatment prescribed by your practice team
Informing the practice promptly of any injury, problem or other matter affecting your oral health
3.2 Missed Appointments
We require a minimum of 48 hours' notice to cancel or rearrange an appointment. A missed appointment fee is payable if you fail to attend a booked appointment or cancel with less than 48 hours' notice.
3.3 Failure to Follow Advised Care
Where you do not attend regular examinations or complete prescribed treatment, you will be liable for any fee reasonably charged for treatment required to restore your oral health that could otherwise have been avoided.
3.4 Termination by the Practice
Where, in the reasonable opinion of your practice team, they are unable to maintain your oral health due to any act or omission on your part, including unreasonable or abusive behaviour, the practice may terminate your contract with immediate effect by giving notice.
3.5 Missed Payments
Where a single monthly payment fails, Denplan will contact you and attempt to collect two payments the following month.
Where two successive payments fail, your contract will be cancelled with effect from the date of the first failed payment.
Any treatment received during a period of missed payment is chargeable in full.
This page summarises the Denplan plan terms between you and The Forge Dental Surgery. The full contract terms are provided upon registration. Denplan administers registrations and collects monthly fees on behalf of the practice; your contract is with the practice, not with Denplan.
